Procedure for obtaining the Metro Reduced Fare Transit ID for People with Disabilities
Please complete the application form by filling in your social security number in the upper right corner and answering all the questions in Part I. Be sure to sign the form. You must also send a photocopy of a proof-of-age document such as a birth or baptismal certificate, passport, driver's license, armed forces discharge, seperation papers or your medical assistance card (if it shows date of birth).
A. If you have a Medicard Card, and are under 65 years of age, you can bring your card and proof of age to either of the LANTA offices below to obtain your Metro Reduced Fare ID card:

You may also mail the completed application to LANTA at the above addresses with a photocopy of your Medicare Card and a photocopy of your proof of age. You will be sent your ID card by mail after the application is processed.
B. If you do not have a Medicare Card, Part II must be completed by a physician, the application along with a photocopy of the proof of age may either be brought or mailed to the LANTA offices.
